I'm looking at buying a Cintiq 16 Pro 2021 version. Does this have the same issue as the older versions with fan noise or has that been fixed in the new version.

Hi, it has. I have previous model, but 21 sound is the same.
I have 2 bent needles on the back with tape to stop the spinning fans. this stop them definitely (you can remove the needles and fasn would spin again)
This workaround has worked for me for a couple of years by now, screen gets hot just in the centre-upper edge. my hand never reach there while drawing so not a problem for me. Other areas in the table are even colder than my old 24" and my 13HD. Bottom edge is cold as if the Cintiq would be OFF
Fans can be very disturbing when having the tablet so close to the head.
Waiting for Apple to release the iPad 15"

Experience may vary but I have found no issue with fan noise with the 2021 version and we have seen a lot less claims in the same time span since the release of both versions.

The area gets hot is so small, that if the Cintiq 16 Pro would be a inch taller, it wouldn't need to have a fan inside.

I was initially interested in a Cintiq pro 16 or 24 but I decided against it after learning about the fan noise and was going to buy a Wacom alternative. But hearing about your modification is making me consider the Cintiq Pro 16 again. Are we talking sewing needles or other kind? Pics would be awesome too if you could post some. Do you get ever get any rattling noises when the fans try to activate?
